Is the problem Repeatable? A repeatable problem is easier to fix than an intermittent one. Does the problem repeat every time? *	Yes No
Write down the exact sequence of events that led to your problem. Write down step by step what you do before the problem occurs. You may want to review the Action Log for an exact recordation of events leading to the problem.
What are you trying to do? Put down in writing what you are trying to do. This will help the Support Personnel to understand your problem.
Are you doing anything else on your computer at the same time as running XRSIM? Document other programs running on your computer.
Rank the severity of the problem: HIGHEST - HIGH - MEDIUM - LOW
